ATTENDANCE

  • Parents are required to sign their child(ren) in and out of the center with their legal name.

  • Sign in/out procedures are in place to monitor attendance. In the event of an emergency, the attendance form is the quickest way to account for the students at the center. 

  • Parents are responsible for delivering and picking up their child(ren) from the Before & After School Care center. No transportation is provided to or from our centers.

  • Parents are responsible for giving proper notification to the Site Coordinator if their child(ren) will not attend the Before & After School Care session. If your child is sick or will be absent, notify the Site Coordinator before the session begins. 

  • Parents are responsible for notifying the Site Coordinator if another authorized adult will be picking their child(ren) up from the center. 

  • Students will be released to authorized adults only. Always be prepared to provide photo identification when picking up your child(ren). 

SPORTS, SCOUTS, AND OTHER SPECIAL ACTIVITIES

SCHOOL AGE CARE
Please notify the Site Coordinator of any additional before/after school activities your child is participating in. All children enrolled in before/after school school sports, boy/girl scouts, PTSA enrichment classes, etc. need to CHECK IN with the School Age Care program BEFORE attending their class/activity and then SIGN IN once the activity is over. 

!MPACT
Please notify the Site Coordinator of any additional after school activities your student is participating in. All students enrolled in school sports and other after school activities need to sign in to the !MPACT program immediately following the activity.

If you move, change jobs, etc. please ensure we have your most current contact information. If there is an emergency we want to be able to notify you as quickly as possible. Complete this form and give it to your Site Coordinator so he/she can update your records.

If you would like to add someone to your child's authorized pick up list, complete this form and give it to your Site Coordinator. Ensure the additional person knows to bring their ID each time they pick up your child. 

Older siblings can pick up younger siblings as long as they are at least 13, on the child's authorized pick up list, and have some type of photo/school ID.
